What Do I Need For My Upcoming Visit?
Please bring your photo identification card, current insurance card, & a comprehensive list of all your current medications. Below you can find two links, one link for patient registration documents & the the other link for the HIPPA & financial documents. If you choose to you, can access these documents prior to your appointment, and print out your registration, HIPPA, and financial documents. If you complete the forms prior to your appointment, bring them with you! This can help us get you seen by the doctors sooner! When filling out the paperwork, make sure to carefully read over each section and fill it out accordingly & accurately to the best of your knowledge. FIRST VISIT
Your initial appointment will consist of a consultation, at this appointment we will listen to you to find out your main dental concerns and get an understanding of what you are looking gain from your treatment with us. During the consultation appointment, you will be evaluated to gain information pertaining to your individual diagnosis and then we will work to provide you with treatment options that address your dental concerns. Many of the patients we see at DIAC have a complex medical history and dental situations, so sometimes it is necessary to complete a more thorough evaluation and bring patient’s back for a second appointment to discuss your treatment plan. Our goal at DIAC is to provide our patients with treatment that fits your individual needs and treatment that will have long term success. Most often, surgery is not performed the same day as the consultation.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any x-rays if applicable
- A list of medications you are presently taking
- If you have dental insurance, please bring current insurance information
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.). If you are currently taking any medications, please bring a complete list of current medications with you to your consult appointment(i.e., heart medications, aspirin, anticoagulant therapy, etc.).
X-RAYS
If your dentist has taken x-rays, you may request to have them sent to our office, please email x-ray images to info@dentalimplantac.com. You also have the option to request x-rays from your dentist be uploaded to disc or thumb drive that you can pick up and bring with you to our office. If additional films are necessary they can be taken at our facility.
Financial Policy
Payment is expected when services are rendered. We accept cash, personal checks, Visa, Mastercard and Discover. We will file directly to your insurance company for any charges incurred during your visit, but payment is expected at the time of service. It is our office policy not to file secondary insurance, though we will provide you with the necessary information to file a claim.
Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Providing quality care is our primary concern. We must emphasize that as a doctor’s office, our relationship is with you, not your insurance company. If there is a difference between my usual fees and your insurance company’s usual fees, you will be responsible for the difference. It is your responsibility to make sure our oral surgeon is a participating provider on your specific dental plan. If you have any questions about any portion of your bill, please contact our insurance/billing coordinator.
